Acne treatment clinical study. Silver nanoparticles against acne.

Evolut silver nanoparticles destroy 650 species of the most dangerous pathogens, including strains that cause acne.

Silver is an ecological antiseptic that have been used by people since ancient times. Modern
technologies make it possible to use this metal in the form of very small particles capable to
destroy bacteria and microorganisms with phenomenal efficiency.

Particles with sizes of 1 to 100 nanometers (10 -9 m) are usually called as the “nanoparticles”. Nanoparticles are active substances that tend to react with any molecules surrounding them. Their high reactivity is a specific characteristic of the metal nanoparticles; which makes it possible to obtain absolutely new substances and materials with essentially different properties. To prevent oxidation or destruction of a nanoparticle, it should be coated with a stabilizer. Russian scientists have developed Evolut – a soap containing silver nanoparticles. To create the soap, a special stabilizing complex was developed that allows keeping silver nanoparticles from reaction with ingredients of the soap composition. All its components and the silver nanoparticles themselves are obtained from natural products and natural substances. The soap has the following composition: cocamidopropyl betaine (extracted from coconut oil), vegetable glycerin, cocoglucoside (coconut oil and sugar), sorbitol, sodium stearate, water, sodium lauryl sulfoacetate, sodium laurate, hydrogenated castor oil, silver nanoparticles, organic orange oil, mandarin essential oil and essential lemon oil. The amber-orange color of soap is due to the natural color of the solution of spherical silver nanoparticles.

ANTI ACNE AGENT ACTION PRINCIPLE

Antibacterial properties of silver from ancient times used to disinfect water in wells. People drank water from silver jars during epidemics and used silver for applications. However, bacteria could not adapt to the silver action for such a long period of time. The production of silver in nanoparticles increases its antibacterial properties by an order of magnitude greater because most of the fact that the atoms of the crystal structure are on its surface; namely, the atoms located on the surface produce a disinfecting action.

Silver nanoparticles destroy the bacterial cell wall causing its immediate death from physical destruction, but not from toxin poisoning, so leaving the bacteria unable to develop a protection mechanism and transfer it to the next generations.

CLINICAL STUDY

Clinical testing of the Evolut soap with silver nanoparticles was carried out on the basis of the Hospital with Polyclinics, Federal State Budgetary Institution of the Administrative Department of the President of the Russian Federation. The purpose of our study was to determine the effect of the Evolut soap with silver nanoparticles on the skin in patients with acne. The treatment group included 15 patients with acne aged 19 to 40 years old consisting of 2 men and 13 women.
All patients used the soap twice a day in the morning and in the evening for a month. The patients were divided into two groups depending on the severity of clinical manifestations and the severity of the course of acne.
The first group (mild acne) included 2 patients (women).
The second group (moderate acne) included 13 patients (the rest women and 2 men). Our study did not include patients with severe acne.

APPLIED METHODS

All the patients underwent a clinical examination, including:
– study of past history and complaints;
– physical examination;
– instrumental examination: photofixation and complex examination on the Multi Skin Test Center MC 750 machine before and one month after the beginning of the soap application;
– laboratory tests: bacterial inoculation for flora including Candida from foci on the skin of the face;
– questioning.

To assess the severity degree, Cook’s acne grading is used based on a quantitative and qualitative assessment of acne clinical elements. The 1st-2nd-3d grades according to Cook’s scale corresponded to the mild severity of the acne course, the 4th-5th grades – the moderate acne, and the 6th grades and higher – severe acne. The efficiency was assessed as:
• clinical recovery (complete resolution of acne elements);
• significant improvement (resolution of acne elements by more than 75% against the baseline);
• improvement (regression of acne elements by less than 50%)
• “no effect” (without significant clinical dynamics).

Cardiff Acne Disability Index (CADI), a questionnaire for evaluation of social dysadaptation, was used. The questionnaire contained 5 questions with 4 variants of answers to each, for which points from 0 to 3 were scored. The maximum score for the test was 15, and the minimum score was 0. The more points, the more acne worsened a patient’s quality of life. The questioning was conducted among all patients before and one month after the start of the soap application.

APPROBATION RESULTS

As a result of using the Evolut soap with silver nanoparticles, we observed active resolution of acne elements. Significant improvement was in 13 patients of 15 in a month after starting the use; clinical recovery was in 1 patient, and improvement was in 1 patient (Fig. 1 and 2). There was no patient who did not respond to the therapy. The Table 1 represents the final evaluation of the effectiveness of using the soap.

The dynamics of the severity of acne according to Cook’s acne grading (Table 2) from the first visit to the last one varied depending on the acne severity as follows: in patients with mild acne, who had single papules (up to 10 pieces) and a small number of open or closed comedones (up to 25), papules completely regressed, and the number of comedones did not changed. Patients with moderate acne can be divided into 2 subgroups. The first subgroup included patients who had papules, comedones and superficial pustules on ½ of the skin surface. At the end of the second week of using the soap, their papules and superficial pustules were almost completely resolved, and the number of comedones decreased by 20-30%.

And the patients of the second subgroup with moderate acne had deep pustules in addition to papules and superficial pustules. Their papules and superficial pustules also were almost completely resolved, and the number of deep pustules did not change, however, a change in the color of the aureola along the periphery of the elements (from cyanotic-brown to pinkish) was observed with decreasing of edema and a diameter of the aureola.

MEASUREMENTS ON THE MULTI SKIN TEST CENTER MS 750 MACHINE

Based on the results of the comprehensive assessment of the skin health on the Multi Skin Test Center MS 750 machine, the corneometry and sebumetry data had diagnostic significance in all patients before and 4 weeks after starting the soap application (Figures 3 and 4). Statistical significance analysis of the changes in these parameters was carried out according to the Wilcoxon T-test (p < 0.05). The “humidity” and “fatness” in patients from the study groups changed insignificantly during the whole observation period (p < 0.05). This suggests that the Evolut soap is neutral with respect to the water and lipid balance of the skin, i.e. it does not affect its barrier properties. The objective data obtained were confirmed by patients with no complaints of skin dryness or exfoliation.

LABORATORY TESTS

We carried out bacterial inoculation for flora including Candida from the foci of the face by the method of time-of-flight mass-spectrometry (MALDI-TOF) with a sensitivity testing to the main spectrum of antibiotics and antimycotics before and one month after the soap application. The growth of microbial flora was suppressed in 100% of patients. Staphylococcus aureus, Streptococcus mitis, Streptococcus epid. and Streptococcus orali prevailed among the isolated microorganisms.
Thus, we can conclude that the Evolut soap with silver nanoparticles has a significant antibacterial effect to the skin.

 

QUESTIONING

An analysis of the results of assessing the psychoemotional state of the patients with acne using CADI showed that following the use of the soap, the state of patients improved (Table 3). The established changes in the CADI indicators, along with the regression of the clinical manifestations of the disease in patients, showed that the quality of life in the majority of them and a subjective attitude of the patients to their disease improved. This is important, since psychoemotional comfort leads to the break of the pathological circle, which plays an important role in the pathogenesis of acne.

CONCLUSIONS

Based on the results of our study, we can conclude that the daily application of the Evolut soap with silver nanoparticles for a month leads to a regression of clinical manifestations of mild to moderate acne without affecting the water and lipid balance of the skin, with no undesirable effects such as skin dryness, burning and exfoliation. In addition, the soap has a significant antibacterial effect. As a result of improving the clinical presentation of acne, psychoemotional comfort of patients is restored. Thus, we recommend using the Evolut soap with silver nanoparticles in patients for acne skin care.
